9/7/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home setting to conduct a family session with the family to introduce QMHP-C to the family. During this time, the family appeared to be delighted to be receiving services as it is a need for the family. Mother further discussed with QMHP-C he really struggles with anger issues, peer interactions, authority figures and just throwing temper tantrums when he wants to. 2. Behavior: Client was cooperative throughout the session as he was listening to the challenges that he experiences within the home, school and community setting which as caused him to have to receive services. He was a little angry with his mother at times within the second portion of the session as well for the mother discussing his behaviors as he would scream to the top of his lungs. 3. Behavior: Client presented normal behaviors within the remainder of the session needing no redirection from QMHP-C. Jahsim was able to discuss with QMHP-C on how he has been trying to be a better child as he did not get in trouble by his mother or his mother’s girlfriend. 1.Intervention: QMHP-C taught that resistance to change in the style of relating to one another can be expected to be high and that change would require concerted effort on the parts of all family members. QMHP-C taught Client effective ways to express his feelings and thoughts in a more controlled fashion without acting out towards his mother disrespectfully. 2. Intervention: QMHP-C actively listen to Client share his thoughts about how he interacts with his mother and demonstrated ways his mother communicates with him. QMHP-C modeled effective decision-making and educated the mother on being less reactive to Client’s behavior when she becomes irate toward his and disrespectful. Through modeling QMHP-C reviewed strategies with Client that would promote positive communication. 3.QMHP-C utilized reframing techniques throughout the session. QMHP-C provided Client with a list of communication styles to assist him with when communicating with his peers and his family members. QMHP-C engaged Client throughout the session through the use of active listening, asking open-ended questions and providing empathetic responses. 1.Response: Client appeared to be in receptive to QMHP-Cs’ interventions and services that were provided to his during the session. Mother reported to QMHP-C via telephone that that it's difficult to deal with Client when he's upset and frustrated. She reported she has learned to be non-reactive to Client’s behavior and not become aggressive verbally and physical toward his actions. 2.Response: Client reported that his mother laughs and jokes with him all the time and when he's angry he becomes frustrated with him begins yelling and screaming. Client reported that he doesn't know when to take his mother serious as he also laughs when he’s upset. 3.2. Response: Client expressed that sometimes he engages in negative behaviors even though he knows the consequences for his actions. Client discussed with QMHP-C that he tries to be a leader when it comes to certain things in the home setting and community setting.
